Make an impact

Food is Medicine programs provide healthy meals, groceries, and nutritionist support to help people manage health conditions like heart disease. The SUPeRFOOD Research Study aims to see how we can help people in cardiac rehab join these programs and make the most use of the resources they provide.

How it works

You will receive a MyChart message with information about Project Open Hand, a Food is Medicine program in San Francisco. You will also be asked to complete surveys after you enroll and 3 months later. These surveys will ask about your eating habits, health, and quality of life.

We take out any information that could identify you before sharing it with other researchers. We will never sell your data or share your identity. To learn more, read our Privacy Policy.
